NOTICE TO ALL PATIENTS:

  • All patients being seen in 2026 need to update their consent and intake forms by clicking here.
  • Please review our 24-Hour Cancellation Policy.
  • Insurance forms for self-submittal can now be done in batches, just let us know what time period you need.
  • Our online scheduler services both our chiropractic and express clinics. Be sure to pick the correct location in the dropdown box on the next screen.

NOTICE TO NEW PATIENTS:

  • When using our online scheduler, you must book a “New Patient Exam” type for everything except massage appointments.
  • To reserve your appointment, all new patients who schedule online are required to have a credit card on file. After you book, our office will contact you at the phone number you provide to securely collect this information.
  • If you are having trouble finding a convenient time slot, try selecting a different provider from the dropdown box.
  • After your initial visit you may follow up with any provider.
  • It is our policy that we DO NOT bill insurance companies for services rendered. Payment in full is due at the time of your office visit.
  • Medicare and Medicaid recipients are not eligible for reimbursement through our office. For all other forms of insurance a self-submitted claim form is available upon request.
