New Patient Scheduling Notice

To reserve your appointment, all new patients who schedule online are required to have a credit card on file.

After you book, our office will contact you at the phone number you provide to securely collect this information.

Similar to reserving a hotel room, your card will only be charged if you are a no-call, no-show for your appointment.

I understand that as a new patient scheduling online, I must provide a credit card to be kept on file to hold my appointment, and that the card will only be charged in the event of a no-call, no-show.
