Review the Terms of Use and enter the six-digit code provided by Davis Family Chiropractic.
This code is exclusive to our patients — ask for it at your next visit or call our office to receive it.
Complete the application form.
Check your email and click the verification link to confirm your email address.
Once we review and approve your request, you’ll receive an email letting you know your account is ready.
