What is a DOT Physical?
A DOT physical evaluates your overall health to confirm you can safely drive a CMV, such as trucks, buses, or vehicles carrying hazardous materials. The exam checks for vision, hearing, and overall physical fitness to ensure you meet federal guidelines and reduce safety risks on the road.
Who Needs a DOT Physical?
Drivers who:
- Operate CMVs across state lines.
- Drive vehicles weighing over 10,000 pounds.
- Transport hazardous materials.
- Carry more than 15 passengers.
You typically need a DOT physical every two years, but drivers with certain medical conditions may require exams more frequently.
What to Bring to Your DOT Physical:
To streamline your exam, bring:
- A list of your current medications.
- Your health history, including past diagnoses and treatments.
- Corrective lenses (glasses or contacts) if you need them.
- Hearing aids, if applicable.
- Your current driver’s license
- Specific medical documents if you have conditions such as:
- Recent surgery or monitored medical conditions: Bring a clearance letter from the specialist managing the condition (cardiology/vascular, neurology, orthopedics, etc.). The letter should confirm your condition is stable and that you’re medically safe to operate a commercial motor vehicle (example: certain abdominal aortic aneurysm cases may require specialist clearance).
- Diabetes (insulin-treated):
- Bring a completed Insulin-Treated Diabetes Mellitus Assessment Form (MCSA-5870) from your treating clinician (valid for 45 days from the date on the form).
- Bring at least 3 months of electronic blood glucose monitoring records (meter/CGM reports).
- Sleep apnea (CPAP): Bring a compliance/usage report showing at least 1 month of documented use—commonly 4+ hours per night with 70%+ usage.
- Missing limb or significant limb impairment: Bring your Skill Performance Evaluation (SPE) certificate/forms if applicable (typically used for limb loss or certain limb impairments—not for progressive conditions).
- DOT drug/alcohol process: If your employer/DOT testing required you to see a Substance Abuse Professional (SAP), bring documentation/clearance from the SAP showing you’ve met the required steps.
Disqualifying Conditions***
A certified medical examiner can (and should) disqualify a driver at any time if a medical condition, disease, or medication could interfere with the ability to safely operate a commercial motor vehicle (CMV). Some conditions are automatically disqualifying under FMCSA standards, while others depend on severity, stability, treatment, and specialist input.
Vision standards not met (visual acuity, peripheral field of vision, or ability to recognize standard red/green/amber traffic signal colors)
Monocular vision (only one functional eye) unless the driver qualifies under the FMCSA Alternative Vision Standard (49 CFR 391.44) and provides the required documentation
Severe non-proliferative or proliferative retinopathy
Ménière’s disease
Uncontrolled vertigo
Labyrinthine fistula or nonfunctioning labyrinth (significant inner-ear balance disorders)
Chronic cough with cough syncope (fainting/passing out with coughing)
Respiratory failure
Requiring continuous oxygen therapy
Cardiovascular disease with risk of sudden incapacitation, such as conditions known to be accompanied by syncope (fainting), collapse, severe shortness of breath (dyspnea), or heart failure
Implantable cardioverter-defibrillator (ICD/AICD) (generally disqualifying)
Narcolepsy
Motor neuron diseases (e.g., ALS)
Acute/active psychosis
Schizophrenia (when symptoms could impair safe driving)
Suicidal behavior or active suicidal ideation
Any mental condition or behavior that may affect judgment, attention, perception, or decision-making
Dementia
Epilepsy / seizure disorder (unless the driver meets FMCSA requirements and/or exemption criteria)
Active/current alcoholism or substance use disorder that impacts safety
Migraines that cause neurological deficits (including vision changes or other neurologic impairment that could affect driving)
Hypertrophic cardiomyopathy or restrictive cardiomyopathy (often not recommended for certification due to risk; typically requires specialist evaluation and depends on symptoms/risk factors)
***Disqualifying conditions that may be appealed:
Multiple Sclerosis
Myopathy
Dyskinesia / Parkinson’s disease
Motor Neuron Disease
Muscular Dystrophies
Neuromuscular Junction Disorders (i.e. Myasthemia gravis)
Peripheral Neuropathies
Cerebellar Ataxia / Benign CNS tumors
