NOTICE TO ALL PATIENTS:
- All patients being seen in 2026 need to update their consent and intake forms by clicking here.
- Please review our 24-Hour Cancellation Policy.
- Insurance forms for self-submittal can now be done in batches, just let us know what time period you need.
- Our online scheduler services both our chiropractic and express clinics. Be sure to pick the correct location in the dropdown box on the next screen.
NOTICE TO NEW PATIENTS:
- When using our online scheduler, you must book a “New Patient Exam” type for everything except massage appointments.
- If you are having trouble finding a convenient time slot, try selecting a different provider from the dropdown box.
- After your initial visit you may follow up with any provider.
- It is our policy that we DO NOT bill insurance companies for services rendered. Payment in full is due at the time of your office visit.
- Medicare and Medicaid recipients are not eligible for reimbursement through our office. For all other forms of insurance a self-submitted claim form is available upon request.
